Use the forms below to submit proof of full vaccination and booster, and to submit COVID-19 test results for on-site rehearsals and performances. Details on vaccination and booster requirements are below.
All musicians are required to show proof of full vaccination as well as proof that they are fully up-to-date on CDC-recommended boosters, based on their eligibility.
Those not yet eligible for a booster will be able to perform so long as they can show proof of full vaccination against COVID-19 (at least two weeks have elapsed since the second dose of a two-dose vaccine or a single dose of a one-dose vaccine).
Booster requirements are:
· Pfizer (for individuals age 12+): Booster must be administered five months after completing primary two-dose vaccination series.
· Moderna (for individuals age 18+): Booster must be administered five months after completing primary two-dose vaccination series.
· Johnson and Johnson (18+): Booster must be administered two months after completing primary one-dose vaccination.
Individuals vaccinated outside of the United States with a COVID-19 vaccine not listed above are eligible, under CDC guidelines, to receive a Pfizer booster so long as they are 16 years or older, and it has been at least six months since they have received all recommended doses of a COVID-19 vaccine approved by the WHO.
In addition to these vaccination requirements, everyone on the premises is also required to wear a properly fitting mask over their nose and mouth at all times except when eating and drinking in designated areas.
